Ethical approval was obtained from Bournemouth University and the relevant ethical and institutional review bodies at the participating teaching hospital. Participants provided informed consent for their data to be used for research purposes and for anonymised data to be archived in an appropriate research data repository. The deposited dataset has been anonymised, with direct identifiers removed and potentially identifying contextual information generalised where necessary. Signed consent forms and other identifiable records are retained securely and are not included in this deposit.
